What is Advance Care Planning?

Advance care planning is the process of thinking about, talking about, and documenting your healthcare wishes in case you are ever unable to speak for yourself.

Having the Conversation

Advance Care Planning, specifically POA-HC, relies heavily upon having a conversation with your health care agent(s). FVAPCP refers clients to The Conversation Project website, conversation guides, resources, and more. The Conversation Project allows organizations to share their links and work for free.
