Madelyn “Maddy” Prailes
Executive Director
Madelyn “Maddy” Prailes is the Executive Director of the Fox Valley Advance Care Planning Partnership, where she leads efforts to expand access to advance care planning and normalize these conversations across the community. She brings a background in nonprofit leadership, program development, and community engagement, with a focus on reducing barriers and improving outcomes for individuals and families.
Maddy holds a degree in Public Administration and Political Science from the University of Wisconsin–Green Bay and is a graduate of Leadership Green Bay and a Future 15 Award recipient.
Outside of work, she enjoys caring for her indoor plants, staying active through walking and pickleball, and spending time with family and friends.

Attorney Jon L. Fischer
Board Member
Attorney Jon L. Fischer is an attorney at McCarty Law, having been a member of the firm since 2013 after finishing both his undergraduate and law degrees from the University of Wisconsin-Madison. His practice focuses on four main areas as he aims to help clients in their time of need. These include estate, probate, Medicaid, and elder law and special needs planning.
Jon holds an Advanced Care Planning Certified Facilitator certificate, conducted by the Fox Valley ACP Partnership in collaboration with Community Care and Bellin Health. Jon and his wife Jennifer live in Appleton with their children.
Jon is an avid outdoorsman and enjoys spending time at his family’s hunting property in Waupaca County. His yellow Labrador retriever accompanies him into the field.
Ben Adams
Board Member
Ben Adams retired from McCarty Law LLP in Appleton in 2021 after practicing with McCarty and with Adams & Woodrow, S.C. in Neenah for 35 years. His legal practice emphasized advance care planning for health care decision making, elder law and special needs planning, guardianships and alternatives to guardianship, nursing home and long-term care issues, and estate planning.
He remains actively interested in many of these areas in retirement. Currently, Ben is a founding volunteer member of the Fox Valley Advance Care Planning Partnership and serves as an advisor to the Board of Directors of the Elder Law and Special Needs Section of the State Bar of Wisconsin.
Additionally, he is an Emeritus Director of the Board of Directors of the Wisconsin Chapter of the National Academy of Elder Law Attorneys.
Becky Reichelt
Executive Committee Member
Becky Reichelt is honored to serve as an Executive Committee member of this important organization that empowers our community and individuals to advocate for meaningful and intentional conversations about future healthcare decisions.
As a senior living and aging services professional, she believes advance care planning is an important aspect of person-directed life, where decisions are based on individual, very personal values and goals.
Currently, Becky serves as the President/CEO of Valley VNA Senior Care in Neenah.
